How It Works

1. Create a form template (or use a system template)
2. Assign the form to a treatment
3. Client books — form link sent by SMS automatically
4. Client completes on their phone; data syncs to their record

Forms are delivered via a secure, hashed link — no login required for the client.

Form Builder

Build any form using 12 field types:

Conditional Visibility

Show or hide fields based on answers to earlier fields. Example: show a "Please describe your allergy" text field only if the client answers "Yes" to "Do you have any allergies?"

System Templates

Ready-to-use templates you can copy and customise:

• Medical history — GP, medications, allergies, pregnancy status
• Hyaluronic acid consent — standard HA filler consent
• Polynucleotide consent — PDRN/PN treatment consent
• Hyalase consent — hyaluronidase dissolution consent

Client Pre-Fill

Forms pre-fill from the client's existing record:

• Full name
• Date of birth
• GP details
• Next of kin

This reduces friction for returning clients.

Reverse Sync

When a client submits a form, answers update their client record automatically — no manual data entry required. Medical history, allergies, and GP details stay current.

Legal Integrity

Every submitted form is protected:

• Template snapshot — the exact form version at submission is recorded (tamper-proof even if you later edit the template)
• SHA-256 content hash — cryptographic proof that the submission hasn't been altered
• Submission timestamp — exact date and time recorded

Cooling-Off Period

Consent forms support a configurable cooling-off period (default: 48 hours). The system blocks treatment until the period expires. This is configurable per treatment.

To waive the cooling-off period for a specific booking (e.g., returning client, urgent case), you can override it from the booking record with a documented reason.

Assigning Forms to Treatments

Go to the Form Assignment tab in Forms & Consent. Select a treatment and link one or more form templates. The linked form is sent automatically after every booking for that treatment.
